I read the article about Prozac (fluoxetine) use during breastfeeding and this is what I got from it, combined with the cautions from Dr. Hale previously, making this basically no different than what we already knew. That is, it is preferable to switch mothers who took Prozac during pregnancy to
something else just prior to or at delivery, because of the long half-life of fluoxetine (2-3 days) and it's active metabolite norfluoxetine (360 hrs!). The article even mentions the slow elimination of these compounds by the infant. The other SSRI's have shorter half-lives and therefore, would
accumulate less in the baby, besides having a lower rate of transfer into milk (this is from Hale's 2002 edition). HOWEVER, if Prozac works better than the others for some women, to the point that it is the necessary med for a postpartum mom, it is relatively safe for the baby per this report.
This coincides with Dr. Hale's L3 rating of the medication, meaning "moderately safe". In short, that means, something else would be better, but this is not contraindicated. Watch the baby.
Did I get that right, Dr. Hale or Frank?
Marsha
