It is fascinating to look at this with my long range eyes of 40 + years and 
I'm sure other long termers like me will agree.  In the 60's-70's everyone 
followed schedules and timing, insufficient milk or perceived insufficient 
milk was rampant.  We went to more on cue and began to see oversupply, GER. 
There must be a happy medium somewhere.

Confounding factors are: mom, baby, stress in   life, mom's style of making 
milk, baby's style of feeding, both of which change throughout the nursing 
period, culture/interference of culture and so on.

I guess the main thought is that no one mother-baby dyad are the same as any 
other!  There are some generalities, but no rules :-)
