The body of a lactating woman is perfectly capable of undergoing surgery, they do it all the time. And I don't think being lactating will hurt her kidney or the receiver either.
I would like to suggest however that she not only donates her kidney, but her milk as wel, both prior to as after her brother gets her kidney inplanted. It will help his body recover and lower infection risk and rejection risk (the latter is a guess).
