I have not worked with Tourette's before. Laid back nursing, side lying where she can have wt on the arm or a ring sling to use with any position or a wrap of some sort so baby is held in place. I do worry about her hitting the baby. If she could restrain the arm and have a ring sling or Moby to hold baby in place she could learn to use the one arm to latch baby. 
Interesting case. I will look more into it.
