Anecdotally, one year I did a "roadshow" during Breastfeeding Week, visiting 
many small towns in our region giving talks on breastfeeding at hospitals. 
Some staff actually listened and followed my ideas.  Years later I met a 
nurse who told me that they had noticed that since they initiated early and 
demand feeding, they noticed that engorgement was rare.
