Hi. I'm in a similar situation, but with a birthing agency instead of a doctor's office. Every place which works with you as an agency does it differently, but they are supplying you with clients and you are using their name, a "cut" taken by the finder (as it was originally their patient) is a given. I never expected anything different when I was hired by this company. 
 
I work for a birthing, doula and lactation agency. I'm an IBCLC and PP Doula and not an RN. My agency contacts me with clients, whom I see in their homes. I do the consultation, (and any follow ups and phone calls) then of course keep  a lactation log with hx and tx options, then a written report and super bill which is all sent to the agency. They take care of billing, and then send me my payment as part of my check which contains the fee they set for me plus a small travel fee. (I also see doula clients.) All the billing and insurance problems are handled by the agency, and they find my clients and deal with any billing complaints, bounced checks, sliding scale issues etc. 

You would have to talk to the office manager or the person who handles your department to see how they would handle your payment and billing situation. But, of course, as they are taking most of the risk and finding clients and dealing with insurance companies a "cut" where the agency is getting usually a bit more than 50% of the fee is the norm in our area. 

I find if preferable to billing my own clients, having to find my own clients, dealing with bounced checks, the client with excuses for not paying, having to explain why I can't deal with insurance companies on my own etc. It also eliminates long "introductory" phone calls in which the client may decide she "doesn't need" a consultation, too as these calls are made to the agency and they do little counseling and leave that to the initial consultation that I do.
