In the ABM news and Views vol.2, NO.l, 1996: a case report by Dr. Anne
Eglash reports that her patient remained on the nifedipine for 3 months.
This pt. did have primary vasospasm (Raynaud's). I think it is important to
stay on the nifedipine until the symptoms decline. Dr.Eglash did go on to
say that occasional mild episodes of vasospasm occurred about 3 times a
week and that is why this mom stayed on it for 3 months.

I personally had a client who also had primary nipple vasospasm with both
her babies and she stayed on nifedipine for many months. With the first
child it took us a while to figure out what was going on. With her second
baby she started the nifedipine as soon as she felt the initial pain and
saw the blanching. She went on to breastfeed for a year. This was a amazing
case as it was a "friend of a friend" and done two states away all over the
phone. I know, not the best way to practice but it worked.

I think it is important to remember that true primary Raynaud's of the
nipple is not all that common. Secondary vasospasm due to yeast, bacterial
infections, or traumatic sucking is more common. A good history will help
tease out what is going on.
