I am looking for a consent for formula use in the hospital for breastfeeding infants whose mother may decline the use of donor milk and choose formula instead.  Does anyone have one they have created and would be willing to share, or know of one that is already in place?  We are in the final stages of being able to offer donor breast milk to not only our NICU babies, but to babies on the postpartum unit as well if they need supplementation for any (medical) reason.  We have a consent form for donor milk, but my director also liked the idea of having one for formula for those moms who for some reason decline donor milk.
