Mary,
Whenever I have seen infants with loud clicking, it was the tongue being
pulled down from the palate, breaking the negative pressure in the baby's
mouth.  Usually this is the result of a short frenulum which "recoils" after
each stretch to the palate.  Sometimes it is tongue weakness, where the baby
'fixes' the tongue to compensate for low muscle tone, then fatigues and lets
go all at once.  The fact that this mom has long nipples just compounds the
problem.
        I have had some success with positioning the baby with the chin
slightly closer to the breast, to bring the tongue forward more; using gentle
sublingual pressure (a finger under the soft area of the chin to give support
to the tongue muscles); and giving really good support so the baby can put
more energy into sucking rather than holding up his/her body.
