Hilary, I had a similar case and what I found was that on a digital exam  the 
baby had an over sensitive gag reflex.  It appeared that the baby was  more 
comfortable or protecting himself from the gag.  The mother never had  sore 
nipples and the baby was gaining well.  The mother stated the noisy  feedings 
were a problem for her when out in public.
I had the mother do some tongue training.  Slowly introducing the  finger 
deeper into the baby's mouth by tolerance along with nursing.  The  baby 
eventually got better but not perfect.
