Hallo Lactnetters!
I'm an IBCLC in private practice in Italy.
Last Sat I saw a 5-day-old baby. He's a 3rd son. 1st sister was 13 months 
breastfed; 2nd sister not breastfed because she did not latch as well. The 
mother had a C-section, after the birth the nurse gave her the baby and she 
said he latched well (?). Then he went for pediatric controls and so on. The 
day after he wasn't more able to latch.
3rd day after birth they went home, the baby didn't latch. Some jaundice (of 
course). The mother pumped milk and gave with a bottle. She phoned at day 5 
telling "he's not able to use well the tongue, exactly like his 2nd sister. 
I'm upset, I dont' want to give formula, I don't want to pump forever". The 
morning after (last Sat) I saw him: the tongue was tied and probably he was 
confused from the bottle too. We tried to give him the breast... not 
possible. The mother has large and pretty flat nipple too. I explained her 
about tongue-tie and speak with her pediatrician. Really lucky that I was 
just the w/e before to a workshop with Catherine Watson-Genna about that!! 
So I enphatized a lot about all the damages of a not-solved tongue-tie. The 
pediatrician was not sure the tongue tie needed to be cut but, as the 
frenulotomy (is it right in English?) is a little little stuff,  finally 
told "ok" to the mother (!). The mother phoned several hospitals, no one was 
able to do that immediately, only waiting for some days. Monday she found a 
pediatric surgeon that did it only because he knows personally the 
pediatrician. He told the mother "well, I never saw before breastfeeding 
trouble for a tongue-tie. In my opinion he doesn't need it" :-//// !!! But 
he did.
The day after I saw again the mother . But, IMHHHO, the tongue was not cut 
enough!!!! With OA he doesn't extend the tongue over the gum .. I told that 
to the mother but told her that perhaps it could be for the bottle too. She 
started to cup-feed the baby. After 2 days, I saw them again. No news. The 
baby is really able to cup-feed, but the tongue doesn't still extend over 
the gum. This evening the mother called me again, she's really upset, she 
told me the baby when cup-feeded drinks too fast and coughs, etc. She would 
like to try with FF.
I will see her tomorrow morning (now it's midnight in Italy :-) )
Do you have ideas in helping this mother?
I think it is really impossible to go again to the surgeon :-///.
is it possible that wating some days while FF he will be more able to latch 
better? I can try to take a photo tomorrow...
