Before it is concluded to be a hormonal problem, you might consider that it's
a pump flange problem.  Women with large flat nipples generally don't fit
into the standard flanges.  Egnell has a flexishield that has helped some of
my moms with large FLAT nipples, and Medela has a large glass shield with an
enormous throat.  You need to look and see if the nipple and areola are
sliding down into the "throat" of the flange.
Also remind her to wet the flange.  At this stage I would also ask her to do
a lot of breast massage and try hand expression.   Good luck.
