Merilee described mothers with low milk supply who seemed not to be
following the plan of care they had worked out with her.

This may not apply, but I have found in working with mothers in that
post-partum period that they often can't handle too many ideas or
suggestions at once. They are tired, hormonal, and often overwhelmed with
learning many other new things. It sometimes seems to help to focus on one
thing - whatever seems to me to be the biggest problem - and help the mother
to change that. (Perhaps it's the latch, or recognizing baby's cues, for
example.) Then I arrange for us to talk or meet again in a day or two, and
at that point I move on to the next step. I make sure I let the mother know,
as I give her this one thing to try, that there are more steps to come, so
that she won't be upset when this first step doesn't magically solve
anything (although sometimes it does).

I know this isn't practical for everyone (I'm one of those volunteer people,
so this can work for me). Perhaps you could go over the plan with the
woman's partner, so that he could remind her of each step?
