Just to keep people updated on the situation right now:

I have offered to go back to see mom & baby tomorrow morning to check
out baby nursing/milk transfer more closely.  I have added a few other
points to the mom, including:

-breast compression
-express some milk (she has to by hand right now; doesn't have a pump)
to give by cup or spoon if baby seems to fall asleep too much and not
suckling well or for very long (i.e. #1:  feed the baby)
-if she is against this, or doesn't have time, or whatever, and the baby
seems to be getting worse, then formula if needed...since she has
already had some in the hospital.
-There is a service here in Québec called Info-Santé (health
information) which I believe is 24 hours/day and I mentioned she could
give them a call.  To be honest, I don't know how this service works
myself, not ever having called it, but it was recommended to me.
-I said it would be better if baby never went more than 3 hours between
feeds during the day and no more than one 4 hour period at night until
we see proof of # wet diapers/seedy poops and definate weight gain.

I talked to 2 IBCLC's today, one of whom is the "closest" to our area
(still an hour's drive).  She can not come down here to see the mom.  If
the mom REALLY needs an IBCLC's help she would have to travel to see one
herself.  :-(  Unfortunately I can't do much about this, except to
stress, if I see the need, the importance of doing so.  :-(  This leaves
me with a situation which, to be honest, I'm not sure I feel completely
comfortable with, only because I am so new at all this.  I feel I know a
lot of the "theory" of it, but as I said to one friend today when trying
to get ahold of one of the IBCLC's, I have, myself, only really seen 3
newborns up close nursing.  One of whom was my own.  Obviously I need to
see more.  ;-)

The mom will be seeing a nurse from her local medical centre (CLSC) on
monday morning, who will weigh the baby.  I have made it very clear that
the mom may call me if she wants to ask any question no matter what, and
that I am perfectly willing to drop in to see her tomorrow and/or monday
if she feels it is necessary.  I do not want to be pushy either though,
and don't really feel like I can do more than this right now.  I think
the next move has to be the mom's.  :-(

As for progress in the last 24 hours:  there was one meconium poop (a
rather big mushy one, but which the daughter of the mom, my friend, said
was still black as tar) last night, and while they were cleaning that
up, the baby urinated.  There was another pee during the night and
another by this afternoon.  Hopefully this evening she may have
completed at least 4 during the day which I still feel is probably not
enough, but like I said, the fontanelle and eyes seemed bright/not
sunken, the lips moist, so my thought for now is that perhaps the baby
just had to cycle a lot of its liquid to the rest of it's body rather
than sending it to the kidneys to be eliminated due to having been under
the bili lights.

Thanks for everyone's tips and thoughts on this one, and I will post
more when/as I know about it.
