I find a good approach to this is to have mom ask the doctor why he or 
she recommends this.

Clomid can markedly reduce prolactin surges so it may trash milk supply 
in breastfeeding mothers. (It works by binding tightly to estrogen 
receptors, which causes the hypothalamus to misread the amount of 
estrogen in the body and stimulate a surge of estrogen, which triggers 
increased LH and FSH, which triggers readiness of the uterine lining and 
ovulation, respectively.) I don't know of any reason that bf once a day 
would prevent Clomid from working. High estrogen levels from oral 
contraceptives can decrease milk production and change the protein 
content of human milk, so perhaps the doctor is trying to avoid that?

Asking is soo much easier than trying to mind-read. And it opens an 
opportunity for dialogue.
