Our Hypoglycemia policy states that infant is to go to breast for a low blood sugar (40 is our low number). Also, a serum glucose is supposed to be drawn to verify the D-stix of 40 or below. This is not always done. Some nurses and docs rush to give formula. If I am notified of the low blood sugar, I help with a feeding and also try to get some EBM to give additionally.

Subject: blood sugars again

 I did not receive any responses to my question about blood sugars, so I will phrase it differently. For those who work in the hospital setting, can you tell me what hospital policy is for ordering formula supplementation for low blood sugar? Are moms given a chance to breastfeed or hand express first? When supplementation is ordered, what volumes are being given? Is there any evidence that volume of supplement has an effect on lowering blood sugar?

I was unable to access protocols/ tool kits from site Margaret mentioned.
These are great questions. I would love to find an algorithm for all the variables one might see such as gestational age, LGA and other risk factors, feeding method and ability, and if symptomatic.
I work in a level 1, and our protocol is to treat if sugar less than 40, but some Peds want to raise the minimum to 50!
I hope we see many responses from this group.

Are you talking about healthy, full-term infants?  Or just asking this
question in general for any situation?  Have you seen the protocol from the
Academy of Breastfeeding Medicine?
