An irish dentist spoke at the LLLI conference  last summer and he was quite
convincing that bottle mouth does not happen with breast feeding.  He had
historical evidence that bottle mouth did exist in our past when all kids
were breastfed (at least those who survived).  Of the number of cavities
found in deceased children, most kids had none, two kids  had decay in their
entire mouth.  Because the statistics were done using teeth counts, it looked
like the cavities were wide spread, when instead these two kids had a horrible
problem and the other kids were sucking through the night, or whatever was
the usual practice, without harming their teeth.

He related caries to Strep mutans and to enamal defects.  Lots of interesting
stuff.
