Currently we chart lactation notes on the common progress notes (as do 
docs, social services, therapists, dietary and just about anyone else). 
We are looking to going to a form just for lactation that can be placed 
on both mom and baby charts and has mostly check offs with narrative as 
needed.

Does anyone have anything like this that they would be willing to share 
or is there anything commercially available?
