My "Breastfeeding Bible" is Chloe Fisher`s "Breast is Best" ideal for the 
professional and for the parents.  I don`t know when it was last updated but 
Chloe is the person who discovered that breast milk does not flow in the 
same constituency throughout a feed.  At the time she ran a walk-in clinic 
at the Radcliffe in Oxford where she was a midwife/lactation consultant. 
Mothers could drop in at any time, even months after giving birth.
She could not understand why mothers would say that they had loads of milk 
but baby did not gain weight and baby clinic said her milk "wasn`t rich 
enough." We have all heard that nonsense before and she would not accept 
that diagnosis.  She had samples of milk analysed in the hospital labs and 
found that sure enough the first flow of milk at each feed was 80% water 
(which baby needs as fluid intake) and that the milk gets progressively 
fatter and calorie rich as the feed continues.
We all know that today and most health professionals (I hope) advise mothers 
to try and empty the breast and then offer the other if baby is still hungry 
instead of the switching that used to be recommended. Chloe was my mentor 
and her book is excellent, informative but readable and in no way fanatic.
