Hi, all:  I'm peripherally involved with a mother who delivered a
32-weeker one week ago, therefore don't know all the details, but I have
a couple of questions.  Apparently the baby never needed a vent, and will
probably be ready for po feeding in about a week.  Mom has been pumping
regularly and is already getting 60cc's at each pumping session, but
wondered why it is still colostrum, i.e. she wonders when "my milk will
come in".  Have those of you with experience helping preterm moms seen a
wider range in the appearance of the milk (I couldn't find any reference
to this in the literature I read)?

My other issue relates to helping mom get baby to breast without having
to go through the intermediary step of bottles first.  The neonatologist
is saying the baby will need to bottle feed first and the lactation
consultant is reassuring mom that nipple confusion is rare in premies,
but in view of Paula Meier's work, it would be preferable to go to
breastfeeding.  This is a determined mother, and she was very happy to
hear that it might be possible to breastfeed from the start.  She
immediately asked me for the articles so that she could show them to the
NICU staff and the neonatologist.  What have your experiences been with
breaking through the institutionalized resistance?  Anything else this
mother can do?  If she were my patient, of course, I could advocate more
directly, but I am simply in the position of providing information.  TIA,
