The article Pam from Zimbabwe was referring to can be found in the American
Journal of Diseases of Children, often abbreviated as AJDC.  To make
matters worse, in the last 4-5 years (can't recall the exact date, sorry),
it went thorugh a major editorial change and also changed its name to now
include adolescent medicine.  If my brain weren't mush, I would tell you
what the new name is. If it comes to me, I will post it for all to see.

The article referred to generated many many letters to the editor which
followed in subsequent issues to the original one.  The journal, by th
eway, comes out monthly.
