Doctor's have a medical diagnosis.
Nurses have a nursing diagnosis.
On my report forms, I list "Lactation diagnosis."  Not my original idea,
don't remember who I got it from, but I love it.

That said, I'm very careful in my wording of hot-button issues. I don't
"diagnose" tongue-tie, but will write stuff like: "Restricted mobility of
posterior tongue affecting baby's ability to create adequate suction and
milk transfer, resulting in poor infant weight gain and reduced milk
production" or "Restrictive lingual frenulum limiting tongue mobility,
resulting in....."   

I will also say, "the most likely cause of XXX symptoms is...."  

Know your providers, know your politics, stand up for your profession.
