Pamela - You are allowed to have pacifiers in your institution. Our NNICU uses them frequently but their practices are not examined by the BFHI reviewers.

For our well babies, we use them for painful procedures like blood draws and circumcisions and then we throw them away.

We instruct breastfeeding families about the risks you mentioned and we encourage formula feeding families not to use them either, so that they, too, are watching for hunger cues. We teach paced bottle feeding and try to get parents not to overfeed with formula.

If families want to use a pacifier we do not provide them; we ask them to bring in their own.

Our staff struggle with requests for pacifiers everyday. We try never to say "we can't give you one", instead the answer could be "your newborn doesn't need one. If he wants to suck, it means he wants to feed."
