I wonder if this isn't when people use it directly out of the
container.  Since it isn't "watered down" it is naturally quite
thick and could block the M. glands as well as not allowing the skin
to "breathe".  I always recommend that mom take just a tiny dab and
rub it between her fingers before applying.  The heat from her
fingers softens it enough that a little will go a long way.
The problem I have is that while it is on the standard order sets,
it is PRN (as needed), but the nurses give it automatically to every
nursing mom.  Then either directly or by implication, the moms think
it will prevent soreness or will the the ultimate cure if they get
sore.  As a result, an uneducated mom may just keep piling on more
and more if she is sore.  When discussing prevention of soreness
with a mom, I try to point out that if she has developed some
tenderness, it will help the healing ALONG WITH CORRECTING THE
ORIGINAL CAUSE OF THE SORENESS.
I do see some moms (often ones with very sensitive skin to begin
with) that will get some surface irritation the first few days even
with "perfect" latch.  I wonder if this isn't the price we pay for
living in a culture that doesn't expose them regualrly to the sun,
wind, etc.  In these cases I make a point that after nursing, the
only change she should see in her nipple is that it will look a
little longer for a moment.  If mom has shape changes (pinching,
"new tube of lipstick" etc.) along with tenderness, then we need to
help her refine the latch.

apparently JCAHO is cracking down on the use of non approved
substances."  Hmmmmmmm....Now this could be interesting.  Didn't we
have some recent discussion that the "new" formulas didn't actualy
have FDA approval?  It would be interesting if that could be pursued
with JCAHO!
