hello fellow lactnetters,
 
i just wanted to update on the prenatal hydronephrosis diagnosis we're  
dealing with. we saw the pediatric urologist today and he did another  ultrasound. 
it looks like the abnormality is the same at 23 wks as 19 wks w/ no  other 
changes noted. he agrees w/ the ped and ob that i can still homebirth and  the 
voiding test can be done up to a cpl months after the baby is born on  an 
outpatient basis in the hospital. 
 
i asked what his recommendation is for prophylactic abx and he says if  
reflux of urine back into the kidney is discovered during testing, he wants  babies 
on abx. but if there is no reflux evident and the kidney is still  enlarged 
w/ extra fluid, he may be willing to wait and see and let me  monitor the baby 
before wanting him on abx.
 
which was somewhat of a relief and not quite the 'abx as soon as the baby  is 
born if there is any extra fluid or dilation at all, even before testing'  
approach my ped uses.
 
my relief was short-lived however as the urologist next told me, 'and i  
highly recommend circumcising your son to help further prevent  infections.'  
yikes! talk about from the frying pan into the fire. i go in  concerned about abx 
and get told to chop off part of his penis!  i tried to  ask if he had any 
knowledge that exclusive bfing can help prevent infections -  intact or not - and 
he said he'd never heard of any studies showing how what a  newborn baby ate 
impacted uti rates. i showed him the references i'd printed  from the 
responses i got here and left him copies, asking him to help me  research the matter. 
i'm not sure how open he'll be to it, but i certainly felt  very supported and 
bolstered in my concerns to have black and white info to show  him.
 
so thanks again for all your responses - both onlist and off - and i  
continue to value and collect every bit of info i get to help back me up in case  i 
do have to show any precedents for not wanting the abx. as for the  
circumcision, i'm not even considering it - i have to b/l the same protective  and 
antibacterial properties in breastmilk that would help protect the kidneys  and 
bladder will help protect the foreskin and urethral opening. 
can you imagine if we told women, "well, bfing might lead to mastitis, so  
you should really consider a mastectomy instead." ???!!!  but i suppose  this is 
a rant for another forum (INTACTNET?).
