Hello from snowless Toronto.   Could someone explain somethings to me about
Vancomycin?  I read in in Thomas Hales,  (1995 edition) " low levels secreted
in human milk" and yet, and this is the part I don't get, the PB = 30%.  If
there is a low PB, why isn't there greater amounts secreted in human milk?
It is poorly absorbed from the infant's gastrointestinal tract, so systemic
absorption is low, however small amounts could affect the intestinal flora.
What is neonatal dosage and how does it compare with 12.7 mg/L. measured in
the blood of an infant of the mom who took 1gm every 12 hours for 7 days?
The rest of the story you can probably guess.  Mom delivered at St.M.'s in
Toronto.  Strep infection.  Prescribed Vancomycin and according to the PHN, 50
people told her she couldn't breastfeed the baby.  Mom is pumping and
discarding and unfortunately using the bottle.  Mom hasn't a lot of money.  It
is important that she breastfeed.  Is there another medication that would be
better?
