Hi Jaye,
I have seen a increase in tongue ties as well. No joke. Last one wasn't
severe, but enough to keep bb from obtaining /sustaining a latch.. Thats
enough for me. If it interferes with bfing then something should be
done...Thats what the peds say. I believe the ABFmed say something similar.
So the only solution was a nipple shield, loads of colostrum lots of swallowing.
Maybe frenulum will stretch. I worked and worked with mom every time bb
woke for a feed. Could get a few sucks in but never stayed with it. so
sucumbed to nasty nipple shield....She knows she needs to be followed. info
given about nipple shields etc.
Back to bb number 3 i believe. mom with no milk.. Had similiar story 6 months
ago. Asked OB to run prolactin. it was low.. tsh normal. I Should have had
him run testosterone and progesterone level. One reason prolactin may be
low for a one in a million would be pituatary tumor..Doctor aware. If the
corpus luetuem is still putting out progesterone/testosterone. (she may have
an ovary with a cyst on it). Hormones competeing with sites. Some women
have residual cysts from ovulation or subclinical polycystic ovaries, which may
indeed affect lactogenesis 11. If the progesterone never drops then lacto 11
would definately be affected. thats why I suggest to test for testosterone
and progesterone levels. Those 4 test should be run. Progesterone,
testosterone, tsh, and of course prolactin. I'm guessing the staff thought she
would have bright red discharge and cramping if retained placenta. I think it is
always worth a look with ultrasound. Non-invasive procedure. Most of the
docs do too. Does she have a fever, foul smelling discharge? Heavy bleeding?
I think that the office staff would be thinking she would have all the other
signs as well. I know from personal experience that is not always the case.
But I'm that one in a million who doesn't have a fever with infection...yada,
yada. Thats all I can think of maybe more brilliant minds than mine can help
you more with this one.

Sorry...hope things turn around for her..
Best wishes,
robin
