Dear Alicia:

        I breastfed my two daughters, both adopted and now in their teens. My son
was born to me 5 yrs before my first daughter was adopted. The first time
we adopted, I pumped for about a month and got only drops of colostrum
before the baby arrived. She was 4 weeks old and had been fed abm by
standard bottle nipple for all that time.  She was taking 4-5 oz 5-6 times
a day when we took her home.  They gave her to us with one little bottle of
abm, and she drank that as we filled out the paper work at the agency where
we picked her up. We had a 45 minute drive home,  the baby started fussing
in the car as soon as we started driving. I put her to my "empty" breast
and she nursed happily all the way home (it was before they had car seat
laws!). I did not use any bottles from that point, just nursed with the
Lact-Aid and about 3 oz of abm in the bag for each feeding. By about the
sixth day, she was leaving milk in the supplementer.  I never did get a
full supply, but was able to discontinue the use of the supplementer at
about 4 months, when I started her on cereal mixed with abm.  I prefer the
Lact-Aid to the SNS for adoptive nursing because it is easy to use for
discreet nursing.  It doesn't show at all once it is in place, and can be
worn between feedings if you are going out. I did not need to bother with
tape once we got used to it. Email me privately if you'd like the internet
address for more info on the device.  With my second daughter, I never
pumped, and my milk came in after about 3 days of using the Lact-Aid.  She
eventually got away from the supplementer, and nursed till age 2 1/2.
I would tell your client not to worry about amounts, just to keep doing
some pumping and do as much nursing with and without the supplementer as
the baby will do when she arrives.  Good luck!
