There are two questions we need to ask:
1)  How do you plan to feed your baby?
We really want to try to assume BF as the norm, and yet we need to find
out where the mom is at in her decision on feeding method.  This would
be our starting point for education.  Any suggestions for better wording
on this one?

2)  We want to identify potential problems (hx breast surgery, inverted
nipples, etc.).  We want to find out the information, but yet we don't want
to imply that if you have these conditions it will be a "problem".
Suggestions?

How about:
1) "Can we answer any questions you might have about breastfeeding?"  or "Do
you feel comfortable that you have all the information you need to
breastfeed your baby successfully?"  Then if she says she isn't planning to
breastfeed, act a little surprised as ask what her concerns are.....

2)  "There are some circumstances under which you might need a little bit of
extra help with breastfeeding.  Let's see if any of the following apply to
you, so we can find out more information if we need to, or get started with
fixing the problems before the baby is born."
