Sue asks about this baby:

"Baby boy-frst baby, term, spontaneous labour, spontaneous birth, no 
analgesia, physiological 3rd stage, skin 2 skin for 90 minutes plus, first BF at 
15 mins-assisted by midwife to facilitate 3rd stage.
Now 5 weeks and 6 days, BW=3460 gms now weighs 4600gms, feeds 8-12 
times per day, wet nappies 4-6 heavy per day, mustard stools one large at 
least once per day, sleeping 5-6 hours overnight.
Mum has 3mm diameter hole punch lacerations dead centre of nipple-and 
nipple thrush. Baby had oral symptoms now resolving with Nystat. Mum's 
symptoms are also resolving with Nystat and supportive measures.
Nipple damage has been fairly constant in last 3 weeks. I suspect this baby 
has learnt to feed well enough for his own needs but to the detriment of his 
mother's nipples. Question how do we educate him to latch better."

How do we know this baby CAN latch better? Have you ruled out posterior tongue-tie? It is very common 
to see thrush with TT. If that is a non-issue, then have you evaluated the baby structurally? Usually when
kids don't latch properly, they are compensating for an inability to do so and my first course of action (if all
of the normal teaching, checking that mom knows good latch, etc) is to get the baby to a chiropractor.
