With two courses of antibiotics, sounds like Candida since both mother and
baby can have no visible signs of thrush and that can still be the causitive
factor for post BFing pain.  What does pain feel like to her?  Or, more
simply, breast pain could be ductal refilling which can cause shooting pain
(BF Answer Book).  I'd lean towards R/O thrush with precise questioning of
Mom and her symptoms.
