We have had a couple babies with mouth sores with the 0.5%, both times it was due to using too much and letting it pool in the mouth.  I instruct moms to paint the inside of the babes mouth and then dab out any excess with a kleenex.  Haven't had any problems with this method.  I did have one very frantic mom call because the baby had latched onto the swab and sucked down a mouthful, scary but no apparent problems.
