Dear all:
Some of you may have ordered superbills from Carol Follingstad who fairly recently passed away. (It may be longer than I remember because I just went into denial mode upon learning of her death because I had talked to her recently over the phone.  She was always so nice over the phone and prompt about sending forms.  During the last conversation she mentioned she hadn't been feeling well.  I never had the opportunity to meet her in person.)  I remember someone announcing this on either Lactnet or the private practice listserve.  I know the announcement came with a note about the contact information for the person who took over her service.  I checked with the private practice list serve first and received a suggestion for someone else's forms, which did not seem to be the same as the superbill only forms that Carol provided and the time frame seemed to take much longer.  Carol Follingstad would usually send her forms out the very next day.  Does anyone on Lactnet know who might have taken over Carol's services?
