Hi,
On the back of our forms that mom fills out w/history and reason for visit etc,
there is a general authorization to treat and to allow communication to moms 
doctor.  This should be on any standardized form you are using.  You could 
make something up.  I don't think you need to talk to a lawyer.  Many lay folks 
don't realize that when they sign in at a hospital for treatment, just what that 
entails.. ie. usually it gives the hosp permission to have the pt seen by 
students and interns among other things.

So, I would use your intake form that you are currently using.  It should say 
something to the effect that I authorize (LC) to assist in my care and 
treatment (or something to that effect) and use my medical info in 
conjunction if necessary as deemed by the LC with my primary physician . I 
am sorry I don't have my form in front of me.  But I'm sure someone on lactnet 
does.  My thoughts are not flowing as smoothly as I would like, but hopefully 
you can get the picture. :) Then the form is signed and dated by mom and the 
Lactation Consultant.

When one goes to the hospital it is understood that touching is part of the 
territory, esp when giving birth.  There is no special clause that allows a nurse 
of doctor to "touch" a pt.  This is understood by the very nature of treatment.
