you may start with the Milk Mob project (www.themilkmob.org) that 
attacks that particular problem getting into the doctors offices 
literally through the back door - educating not doctors (who 
unfortunately too often lack both time and desire to learn breastfeeding 
skils), but theirlicensed (RNs) as well as non-licensed staff (MA's and 
so on).

Hanging a carrot of improving patient satisfaction with the paid for 
training may lure physician ofices into this training... Hopefully... At 
least that is what I am trying to do despite of all the resistance (grrr)
