I'd be more concerned that there is "no milk" rather than "no MER",
though I have had moms with such severe engorgement that they have had
trouble pumping at first. Jean Cotterman's Reverse Pressure Softening
technique is invaluable is mom is really engorged, so is gentle breast
massage during pumping. If both of you are comfortable with "hands on"
helping, and RPS is not enough, you can alternate ice packs for 3-5 mins
with gentle massage while she pumps.
What is her health history? Any thyroid problems, PCOS or other
hormonal imbalances that might reduce milk supply? Sometimes moms with
PCOS report feelings of engorgement, but there is really very little
milk there. Did you palpate the breasts?
Letting the baby bf for 30 mins and only take .1 oz is a recipie for
disaster, as you well know. This little one is gonna run out of energy
to keep working that hard for so little.
