Nikki,
You mentioned that "The mother has tons of milk, ....... and adds
some of her cream to her milk."
Wouldn't this be equivalent to the ped's suggestion that she use the
higher calorie formula?  Especially since this infant has already
shown some reaction to formula (when mom added powdered formula to
the EBM) switching would likely only add to the problems.
As you said, Downs babies have trouble gaining no matter what they
are fed.  The fact that baby already has cardiac problems and
pending surgery only adds to the gain problem.  I wonder if some may
need gavage feedings to be able to take enough in.
