Hello,
I remember back at the ILCA conference when discussing some possibilities of 
no/decreased supply was that some women develop normally with milk in the 
breasts but for reasons not yet known, the milk ducts never make it to the 
nipple to let all that milk out.   It could explain the breast pain and maybe she 
is making milk it just cannot get out.  Could you have her tested for prolactin 
levels?   Progesterone levels and androgen levels?  This is just a guess thrown 
out there.  I hope you can help this poor gal..  And baby who is missing out...
