Greetings,
In the hospital I previously worked at we needed an Rx from the pediatrician or 
the OB in order for insurance to cover the consult.  We only did inpatient and 
not home visit as generally the private practice or public health did that.  It 
was always my understanding that if an RN did the consult then it was eligible 
for reimbursment(in hopsital) along with the Rx.

Sounds like you have a wonderful opportunity to develop a good Lactation 
Clinic and support for those in need.  Good Luck
