Just pondering here -- it's certainly a question to explore, because adding in active hormones for long periods could have plenty of unintended consequences.  But the high androgen levels, prompting the early use of birth control pills, seem a more likely candidate for disrupting the normal sequence of breast growth.  The birth control pills' create estrogen/progesterone levels that make the body think it's already pregnant -- women sometimes use heavy-duty birth control pills as part of the preparatory regimen when they are inducing lactation, to encourage breast growth.  Since we deal with so many mothers who are having their first baby at age 40, we forget that what we think of as "a teen mother" was for most of human history just "a mother."
