Chayn,
It is my standard protocol that every single baby who has a TT is referred for CST. I have moms
schedule an appt for the same day or next day from the clipping. If the clipping will be delayed more than
a few days from when I first see them I have baby get CST before the clipping as there is often enough
improvement to manage breastfeeding until the clipping is done. I see no benefit in waiting for CST and
think it is a miracle than any baby with a TT would ever achieve full function w/o the CST.It is certainly
true, however that nursing the baby often reveals remnants of tissue that need to be clipped, but then so
does the CST w/o waiting for 2 weeks.

Dear Wise Ones,
Please Help. I have worked with several women who took their babies for
frenotomies, felt marked improvement in breastfeeding for about two weeks
and then their babies went back to their old, pre-frenotomy latching and
sucking. Many of them go back for a second clip and in at least one case,
the same thing happened after each clip (three times! - at which point she
called me and I suggested CST and no more clipping) Its gotten to the point
where I hold my breath for 2 weeks after one of my clients takes their baby
for a frenotomy - even when the mother is thrilled with the initial
results. One of the doctors who is considered an expert in the area, warned
one of my mothers that the baby may regress after 2 weeks though he did not
tell her why. I did not find any information on this phenomenon.
Does anyone understand why the babies do this and what to do about it?
Thanks so much,
Chayn Fogelman IBCLC Israel
