I rent Ameda brand hospital grade breastpumps, sell only brand new 
presterilized kits, and wipe down the pumps with disinfectant when they 
are returned.

When I sell a consumer grade pump (Purely Yours), I explain to the 
mother that she needs to wash and boil the kit (but not the tubing since 
Ameda kits are a closed system) before the first use. Since these pumps 
are purchased for returning to work or casual use, there is no need for 
me to presterilize the kits, the mom has time to do that herself.

Pump In Style pump motors are not sterilizable, they have no filters and 
therefore organisms could *theoretically* get into the motor and be 
blown back through the tubing into the collection chamber.  Moms are 
advised to wash and sterilize the kit and tubing themselves when they 
get home.  I strongly doubt that this mom's mastitis had anything to do 
with an organism in the plastic kit that came from the factory.  It is 
more likely that the mastitis was partly due to using the PNS for 
something that it was not designed for - like clearing initial 
engorgement, or completely maintaining a milk supply in the absence of a 
breastfeeding baby.  What was the exact situation?  Perhaps you can 
explain to the physician which pumps are designed for which conditions 
of use, and that asking a pump to do what it was not designed for can be 
problematic.
