I have to say that if I were that mother, I would change obstetricians.I 
think if I were the LC in that scenario, I think I would offer that as a 
suggestion as well.

Maybe she can find another more breastfeeding-friendly OB practice or a 
nurse midwife skilled in this area.

The plastic surgeon is probably NOT the appropriate physician to ask 
anyway. Although he is poorly educated in breastfeeding issues, that's not 
likely to change as breastfeeding probably isn't in his area of interest 
(not to mention expertise).
