The book is "On Becoming Baby Wise" by Gary Ezzo & Robert Bucknam,
published by Multnomah Books.  They indicate for information to write to
Questar Publishers, Inc., P.O. B. 1720, Sisters, Or 97759.

I just received this book from a client who has always had trouble
responding to her kids (in my view).  Both babies lost a substantial
amount of weight in the first week and never gained well. She is very
happy with the system in this book although it wouldn't work for me.
I'm trying to read it to know what it says, but their approach is not
oriented in the direction of my parenting.
