Kathy,
Our hospital, deliveries 4,000/yr and a NICU for Central MA, has a policy
that if the BS is below 40 the infant is breastfed.  The accucheck is
redone 1/2 hour after feeding is finished.

We used to have to feed formula, but three years ago  with the help of
the Breastfeeding Resource Team and the LC at that time the physicians
routine orders were rewritten.   This has been very successful for us.
