Maybe it's the mag sulfate that is causing the delayed lactogenesis.  I've
seen that before, and it seems to me I've read it in the lit.  I'll look for
a ref.  I think the management you are doing is appropriate.  Just keep
feeding baby and stimulating the breasts and milk should show up. It may
trickle in and take a while to build, so taper off the formula rather than
cold turkeying.
