In my opinion, the situation you describe is a medical emergency. If the ER
doctor saw fit to give IV fluids, that is saying something, no? A baby w/ a
b.wt of 8#3 and now 6#12, who is still lethargic and not feeding well at
10-11 days pp, has something seriously wrong. I would advise the mother to
go to a different ER if possible, is a children's hosp available? Or go to
another pediatrician asap. The baby can be fed IV and/or by gavage if he
cannot feed normally. He should be taking 20 oz or so per 24 hrs (give or
take a few). This baby could surely be septic, have a heart defect, or some
metabolic disorder, to name a few things.
Surely the mother should, if possible, do what she can to increase and
maintain her supply (by all the usual measures) but that is secondary to
getting help for this baby. You and the mother are right to be worried.
