I had the same problem with a patient who had actually started pumping to relieve the discomfort ( she was 20 weeks pregnant at the time!!!!) I too suggested ice packs and firm bra. Needless to say, I discouraged the pumping!
This mom delivered today and she told me that these remedies solved her problem, unsurprisingly, the baby is nursing wonderfully well - lots of colostrum.

Hello all!
I rarely come into contact with this situation. I have a G2L1 mom at 26 weeks

complaining of severe breast engorgement and "lumpiness"; no signs of

Mom is currently using ice packs and reduced stimulation, proper-fitting bra.
After discussing with her CNM, we both agreed trying Evening Primrose Oil was
not yet
suitable due to her gestational age (we wouldn't want her to efface or
dilate too soon).
I discouraged cabbage use also at this time (we don't want to dry up- just
relieve
engorgement).
Any suggestions? I will be doing a full evaluation tomorrow. Thanks in advance
