Our entire hospital (all charting except for docs-soon to change) uses
computerized charting. We have developed a lactation inpatient (new and
follow-up) patient form and an outpatient form (new and follow-up) which
imports basic data automatically, has choice lists and ability to free text
as well.  It is a combination charting form and database (some items).  It is
not printed out unless the doc or mom need it for some reason. Anyone with
authorization (computer password) can access it.  Our hospital system is
"Clinicomp".
