First thing that came to my mind was, Is mom supporting her breast for
this baby?  I have seen many moms who latch baby on and then let go of
breast.  Baby will either fall off or slide to the tip of nipple.  Her
breast size can help you determine how crucial it is for her to mantain
the support of her breast.  Also, is the weight of the breast on the
baby's chin causing baby to fall off? A dancer hold may help.
